Why does healing feel like going backwards?

Because it partly does, in the short term. Recovery is non-linear, and setbacks are a normal feature rather than evidence of failure. There's also a perception effect: as you improve, your standard rises, so an ordinary bad week feels like collapse compared to how you've recently been functioning.

How do I know if I'm actually healing?

Look at recovery time rather than absence of pain. The reliable marker isn't that bad days stop happening — it's that they get shorter, and that you return to baseline faster afterwards. Most people miss their own progress because they're watching for the pain to disappear entirely.
